Immunization Abbreviations
HepB Hepatitis B, a serious liver disease   MMR Measles, Mumps, and Rubella
DTaP diptheria, tetanus (lockjaw), and pertussis (whooping cough)   VAR Chickenpox, also called Varicella
Hib Haemophilus influenzae type b, a serious brain, throat, and blood infection   HepA* Hepatitis A, a serious liver disease.  This vaccine is currently a recommendation, not a requirement for Ohio.
IPV polio, a serious paralyzing disease   Tdap* Adolescent/adult booster for diptheria, tetanus (lockjaw), and pertussis (whooping cough).  This vaccine is currently a recommendation, not a requirement for Ohio.
Rotavirus* Rotavirus, a highly contagious diarrheal illness.  This vaccine is currently a recommendation, not a requirement for Ohio.   MEN* Meningitis vaccine for adolescents/adults, a serious brain and spinal cord infection.  This vaccine is currently a recommendation, not a requirement for Ohio.
PCV Pneumococcal conjugate vaccine protects against a serious blood, lung, and brain infection.  This vaccine is currently a recommendation, but not a requirement for Ohio.   HPV* Human Papilloma Virus (HPV) Vaccine is routinely recommended for girls age 11-12 years old.  It may be given to girls and women age 9-26 years old.  This vaccine is currently a recommendation, not a requirement for Ohio.
FLU*
(Annual)		 Influenza, a serious lung infection.  This vaccine is currently an annual recommendation for age 6-23 months, but is not a requirement for Ohio.
